I've been trying both Streamlabs OBS and OBS Studios but nothing gives a good quality.

Ironically the quality of recording was much better back in the days.

I guess this has to do with BattleEye today or something, since these recording programs has to use some anticheat-hook to work for tibia so you don't get blackscreen.

I think even the biggest content creators of both making videos and streaming Tibia, like Itexo for an example has suboptimal quality.

I mean you can see everything that happens but it is ALWAYS a bit blurry.

Is it even possible getting an almost "perfect" quality these days?

Excellent question but it should be addressed on the topic above. I believe that you can comment there about your perceptions and even use the comments to inquiry answers there about this issue. You can also make another question asking if such thing as a modern Tibiacam or Tibiacast exists, I think, since they worked in a different way compared to a Camtasia or even Fraps. The trick there was that they recorded the commands only in order to record and play the Tibia game instead of recording your display (which means lighter archives as well). Regarding recording programs, you can find a bunch of these in the topic mentioned above. Getting an old Fraps to record modern Tibia will most likely produce the same result that you are describing.

I'm not sure what kind of quality you are looking for. I use OBS Studio and I think I may know about the blurry effect you are talking about. I haven't changed settings in a long time so maybe I could improve the quality a bit.

I have no idea what kind of PC you have or what settings you have configured for recording. The recording quality can depend on resolution, encoding, quality settings, and other OBS settings. You may have better luck posting directly on the OBS forum with more details of your current configurations.
