Why does a creature lose hit points even if I'm not attacking?

Hello guys,

iam back to the game and I see often that monster losing hits without getting attacked.

Like 32 hp / 2 seconds.

Is it a charm or item upgrade?

Are you using any item with a damage reflection attribute? Like the Eldritch Shield, Spirithorn Helmet/Armor, or Eldritch Crescent Moon Spade?
