Your question is very interesting because when I found Omrafir for first time, I was around level 400 ek. In that case, I was wondering that should I go downstairs and he will kill me instantly or I can tank him without any issues.
TACTIC:
The boss is very simple. The hardest part is positioning him properly in the corner. During that process, if you line up with the boss, you can get hit by the Great Energy Beam (not Fire Beam, as you mentioned), which deals around 9k damage — meaning you die in one shot.

As u can see on picture, when you place boss in the corner, you should stand diagonally to him, because you can easly die by GEB (blue arrows). The team’s job is to keep magic walls/bushes as u can see on picture that the boss won'tt leave the corner and the most important thing - NO FIRE DMG, paladins CAN'T get summons!!!
6750 HP (level 445) + 1350 HP (3 gems) = 8100 HP
In theory, if Omrafir “hits hard enough” you can still die in a single hit, which is why proper equipment is crucial.
Equipment:
Spiritthorn Helmet – +10% energy protection | +6% physical protection
Falcon Plate – +4 shielding | +12% physical protection | +15% energy protection (imbued)
Stoic Iks Cuirass – +2% energy protection | +4% physical protection
Pair of Soulwalkers – +7% physical protection
Gnome Shield – +2 shielding | +8% energy protection | +6% physical protection
Ink Blade – +2% energy protection
Prismatic Necklace – +10% physical protection | +15% energy protection
Prismatic Ring – +8% energy protection | +10% physical protection

Of course, this setup is heavily focused on energy and physical protection and it is more than sufficient. It all depends on what eq you have — you can definitely put a “cheaper” version and still tank it successfully. :)
If u have almost full BiS set you don't have to worry about physicall dmg cause it will hit u max 4/4.5k HP.
About Fire Dmg it's more dangerous than GEB. Advice is the same -> Wild Growth/Mwalls on the boss u stay diagonally. I read something about Omrafir and he spells quite often Fire Wave than GEB so better equip set on fire (Soulwalkers, Stag Legs, Falcon plate with fire imbue, Sun Catcher) - I think that eq is enough cause if boss has 12k max dmg by fire, your set will give u 32% of fire protection -> +/- 4k dmg = 12k - 4k = 8k (far enough). Key is in skills and quick reaction - u see that Omrafir can move, u move faster than he can rotate to make hit from beam/wave.
Summary:
With the above equipment and a solid team, you should be able to tank Omrafir at level 445 without major issues.
